I'd like to make a complaint for someone else
You are about to start the process of making a complaint.
We take this very seriously, so please provide as much information as you can to help us with our investigations.
We'll ask you for:
- your contact details
- details about the person you're making a complaint for, including their date of birth
- what the complaint is about, including times, dates and who was involved
- the details of any witnesses
Please note: we can only investigate a complaint made on behalf of someone else when we have written consent to do so from that person.
